I am offering my services to people with type 1, type 2 and gestational diabetes, to assist in self management of diabetes, in and around the inner eastern suburbs of Melbourne,
As diabetes is a chronic condition, people with diabetes are eligible under Medicare to see allied health providers at a reduced rate.
The GP develops a care plan including an Enhanced Primary Care referral.
Blood glucose monitors are supplied at no cost and recommendations on the frequency of testing is provided.
